Our providers ensure our clients' providers are communicating. They also assess our clients' care plans to prevent gaps, duplication, or unnecessary costs of care.
Through this method, we're building personalized care plans and ensuring their needs are met, improving client outcomes with fewer hospital visits and overall better quality of life - keeping them independent and safe in their homes.
Clinsura™ provides home-based care coordination services with our experienced staff of coordinators.
We also train mental health counselors and provide counseling services through our partner, Solsura™.
We work with our clients to coordinate their care, from health care providers, to social services, to mental health providers, we work with our clients to close care gaps and provide better outcomes.
